1. Complete the online consultation form
This form starts the consultation process by collecting information about your pet. Only ONE pet’s information PER FORM.
2. Veterinarian Referral
Once you complete the form, an automated email will be sent to your primary care veterinarian. They complete the referral form and submit medical records.
3. Payment Confirmation
These fees vary based your pet’s specific needs. Once payment is confirmed, we proceed to schedule your appointment.
4. Consultation Appointment
Book your consultation with Dr. Chae. Discuss your pet's nutrition needs and create a tailored plan for their well-being.
